It’s one of those things that if you share with a normal Democratic voter, they might not believe you, but it’s happening. Longtime Democratic congressman Al Green of Texas joined local LGBTQ+ advocates Thursday in calling on the Department of Homeland Security to restore hormone therapy and other specialized medical care for transgender illegal immigrants held in ICE detention facilities.
Green appeared at an outdoor press conference with Brandon Mack of the Houston LGBTQ+ Political Caucus and officials from the Montrose Center. He read from a Sept. 4 letter to Homeland Security Secretary Markwayne Mullin, arguing that recent agency policies have created unsafe conditions for transgender detainees.

“Dear Secretary Mullin, I write to express my deep concern over reports of hazardous conditions for transgender immigrants in Immigration and Customs Enforcement detention facilities, resulting from deliberate agency policies,” Green said. He said those policies include “the denial of medical care and removal of protections for transgender individuals.”
Green said specialized medical care, including hormone treatment, had been halted at least 10 detention centers. He argued that the change could cause physical and mental-health complications.
He also criticized the end of a Biden-era practice that reserved certain housing units for transgender women. Green said those detainees are now housed with men or placed in isolation.
“Transgender persons deserve to live without fear that their identity will be used as a reason to deny them care, especially health care, safety, or humane treatment while in ICE custody,” Green said. “Government custody must never become a license to discriminate.”
Green compared the issue with medical care provided to bank robbers or enemy combatants, arguing that people held by the government who have not committed crimes should receive needed treatment.

Daron Perez, TransWellness program coordinator at the Montrose Center, said detainees should not have to wonder whether they will receive hormones or whether their identity will be respected.
“Being detained should never mean losing your dignity, your safety, or access to necessary medical care,” Perez said.
Mack and Montrose Center CEO Avery Belyeu also called for continued access to medications, separate and safe housing, and an end to what they described as discriminatory treatment, according to Breitbart. Green linked the issue to his own experiences with racial discrimination, saying people should not be punished “because of who they are.”
The Trump administration has opposed providing hormone therapy to immigration detainees. In August, DHS said it would “NOT waste taxpayer dollars to provide hormone therapy to illegal aliens seeking to change their sex.”
Officials have cited President Donald Trump’s January 2025 executive order directing federal agencies to recognize sex as an immutable biological classification and to base detention policies on biological sex. ICE has said it provides comprehensive medical care, including emergency services, and uses segregated housing only for health or safety reasons.
Neither Mullin nor ICE immediately issued a public response to Green’s letter or Thursday’s press conference, which was streamed on Green’s official YouTube channel. Green’s office announcement confirmed the event and the participation of the Houston LGBTQ+ Political Caucus and Montrose Center.
